Not Getting Better After Months of Physio or Chiro? Here's How to Tell If It's Working

If you've been seeing a physio or chiropractor for months and you genuinely can't point to any progress, that's not just discouraging — it's a clear sign. Pain can fluctuate day to day, but a lack of measurable improvement over weeks or months means something about the approach isn't working for you.


Why "No Measurable Progress" Is a Red Flag


It's normal for pain to have good days and bad days even when treatment is working. What's not normal is going six to twelve months without being meaningfully closer to your goals than you were after the first month. Outside of a small number of severe injuries, most musculoskeletal issues respond to appropriate treatment well before that point — if they're not, continuing the same approach indefinitely usually isn't the answer.


How to Evaluate Whether Your Treatment Is Working


  1. Track Objective Markers Every 2 Weeks - Range of motion, load tolerated, frequency of flare-ups — not just how you feel that particular day.

  2. Compare to Your Baseline at 4-6 Weeks - You should be able to point to specific, measurable change by this point in most cases.

  3. Account for Genuine Exceptions - Severe injuries repaired surgically, like ACL ruptures, legitimately take 6-12+ months and shouldn't be judged by the same timeline.

  4. Seek a Second Opinion at 6-8 Weeks of No Change - If your tracked markers haven't moved by then, it's reasonable to look for a different approach or provider.


Signs Your Treatment Is (or Isn't) Working


  • Working: You can point to specific improvements in load, range, or pain frequency over the past month.

  • Working: Your provider adjusts the plan based on your progress, rather than repeating the same approach indefinitely.

  • Not Working: You've made no measurable progress in the last couple of months.

  • Not Working: You like the person you're working with, but liking them isn't the same as getting results.


You can respect a provider and still recognize that their approach isn't getting you anywhere. If the data says you're stuck, it's worth finding someone whose plan can actually move you forward.
