When physio exercises don't produce results, the default explanation in many clinical settings is compliance: the patient didn't do them. This narrative is convenient and common. It's also usually wrong.
The real reason home exercise programs fail has almost nothing to do with willingness and everything to do with how exercises are prescribed, taught, and communicated.
Why People Don't Do Their Exercises
The three primary reasons people don't follow through with home exercises are:
They weren't taught them properly — watching a clinician demonstrate something once in a busy clinic isn't enough for most people to replicate it at home, especially for exercises requiring proprioceptive cues
They didn't understand how the exercises connected to their goals — "do this band exercise 3x10" is easy to deprioritize when you have no idea why it's supposed to fix your shoulder; people consistently adhere better when they understand the mechanism
The instructions were unclear or the progression was missing — many programs are handed to patients as a static list with no guidance on when to make it harder, how it should feel, or what counts as progress
Why Exercises Themselves Often Fail
Even when patients do complete their exercises, results often disappoint because the program wasn't adequately designed:
The exercises are too easy to drive any meaningful adaptation
They target isolated muscles rather than the movement patterns actually causing pain
They don't progress in load or complexity over time
The clinician prescribed the same protocol they always prescribe, rather than something specific to this patient's mechanics
