Is It Safe to Lift Weights If You Have Scoliosis?

If you've been told to avoid exercise because of scoliosis, you've likely been given outdated advice. The question "is it safe to lift with scoliosis" is one we hear regularly at the clinic — and the honest answer, for most people, is yes. Here's why the opposite belief is not just wrong, but often makes things worse.
What Scoliosis Actually Is
Scoliosis is an abnormal lateral curvature of the spine — an S or C shape when viewed from behind instead of a straight vertical line. It typically develops during adolescence and is measured by Cobb angle. Most diagnosed cases are mild (under 20 degrees of curvature) and remain stable throughout adulthood. Severe cases, above 40 degrees, require closer management and are significantly less common.
Does Scoliosis Cause Back Pain?
Not automatically. Many people with mild scoliosis have zero pain and never will. The curvature itself is often not what's causing symptoms. What commonly creates pain in people with scoliosis is the deconditioning that follows being told not to exercise. When the muscles that support the spine weaken from inactivity, the spinal structures are left to bear more load without adequate support — and that's when pain develops.
Why Exercise Is More Important, Not Less
The muscles surrounding the spine act as a secondary support system for the vertebral column. Strengthening them reduces the load the curve itself has to bear. This is especially true with scoliosis, where asymmetrical spinal loading means the muscles need to be strong enough to compensate and stabilize. Regular strength training also improves proprioception — the body's positional awareness — which supports stability under load.
How to Start Safely With Scoliosis
Begin with low load and high control: prioritize technique over weight, especially early on
Focus on posterior chain: deadlifts, rows, and carries build spinal stability without aggravating the curve
Learn proper bracing: a solid bracing pattern during lifts reduces compressive and shear forces on the spine
Work in a pain-free range: some asymmetry in loading is expected — monitor for movements that reliably produce symptoms
Increase progressively: as strength and motor control improve, load can be progressed like any other program
Signs Worth Investigating
Pain that consistently increases with a specific movement warrants a professional assessment. But "I have scoliosis so I can't train" is not a useful starting position — it usually just delays strengthening the spine when that's exactly what's needed.
